What information should be included in the power of attorney?

Please fill out the information below about the delegation and agent. 

  • Delegate (Name): Name of the representative who did not authenticate himself/herself at the maker studio
  • Business registration number: Business registration number listed on the business registration certificate
  • Address: Business address listed on the business registration certificate 
  • Agent (Name): Name of the representative who has completed the studio identification
  • Wadiz Member ID: Email registered at the time of Wadiz membership registration
  • Birth date: Agent's date of birth
  • Home address: Agent's home address
